BMS naming convention is NM for normal scale charts and no prefix for insane scale charts or at least thats how I have seen it be done historically. For example, here's an old thread from a pack of hundreds of BMS maps with the diff names explained https://osu.ppy.sh/forum/t/721917?n=1. Also the largest BMS packs on osu which contain 1000+ BMS converts use no prefix for insane scale charts.
Or, if you want a more quaver-friendly excuse: "The highest difficulty of each game mode is always free to have a custom difficulty name"

I have just put this mapset on hold, which means that minor issues have been found in the mapset and we are waiting for a fix with a map update. Please fix the listed errors and update the mapset.

Change difficulty names to show clear progression in difficulty. In the case of levels, just dont add it for the middle one, you could just do like NM Lv. 10 -> HD -> IN Lv. 03

Isn't required, but maybe just keep the middle diff name consistent with the naming scheme of the others

You need another difficulty in the spread, the gap between them is a little too big!
